Michele is very concerned about my over all health, and shows it each time we talk. She is professional and very thorough in our visits.
Coach K always makes sure to listen to me and any concerns I have, while also giving great advice on what I can do to improve myself and my health. In today's world of just being another number to many health professionals, she relates with you and lets you know that she's listening. I truly appreciate her.
I had a heart attack in Feb 2024. And I’ve never experienced any major health event prior to my HA. Everything felt overwhelming, and being able to speak with my Community Wellness rep (April K) has been really beneficial in navigating my post recovery process. It’s important (for me) to have a wellness partner that I can share concerns and learn about asking questions that I didn’t know to ask.
What a great program! When I returned home after my stroke I was worried about what would happen to me. The Community Wellness program was right there helping me from day one. They check on you as deemed necessary and monitor your progression. I was given a blood pressure machine that connects to the programs App immediately giving those monitoring an instant reading. I have come along well due in part to these wonderful people. Thank you Community Wellness.
My care team is quick to check on me when my vitals are abnormal. They have called quite a few times when my blood pressure has been low. Heather is very caring and attentive to my health and needs. I think she is a wonderful nurse!

My wellness counselor, Carla P., is always happy and cheerful. She is very easy to talk to on many things but most importantly my wellness. Things like my health goals, also life changes that I can make to supplement the long term medications I take for my health conditions and suggestions to aid in my recent crazy work travel schedule. I look forward to our appointments every month and always leave our meetings engaged and positive about making a healthier me. 🤓

She's always politeand nice. Very professional. And she puts up with me n I'm probably one of her hardest clients.
I feel like she really cares. She is so kind. Understands so much. I literally felt like I've known her forever.
Overall I think the coaches and on duty nurses are great. I am a little unhappy with the equipment however. Often times the reading on the device is not the reading that registers on the phone so I end up putting the device reading in manually. My equipment has been replaced and it still happens.
It's been very helpful for me to have someone checking in on my health recovery.
Great service
At first I was not to impressed or excited to be apart of the program itself. I wanted to use it to be able to come home and not stay in the hospital for a long period of time. But being able to track my progress with managing my conditions by monitoring my vitals, and talking with professionals helps me get through it all. All of the advice is excellent to keep and the people helping are always nice and supportive. I am a happy customer.

Yes, I love April to pieces! She’s thorough, helpful in anyway needed, finds and sends me links to things I can try or read about. Easy to get an appointment with for next month. We always go through my meds and questions. I couldn’t ask for a better coach!

This app and the people involved have been wonderful. Carla Phillips has not only been emotionally but also medically necessary. She has helped me understand my test results gave me information to help me be healthy. She has talked me through the emotional struggles in my personal life that seemed unbearable. But she's not the only one. The nurses call me immediately if my vitals registered are out of bounds, this app has been a complete eye opener to my lifestyle and the effects of my health. Thank you who ever developed it. I recommend it to everyone and tell everyone about it. I've been using it for almost a year
