I am so thrilled that I joined Form. Over the years I have tried many things to lose weight but nothing ever stuck. The amount of support received and the convenience of online appointments have made me successful! When I am feeling frustrated, I remind myself it’s a marathon and not a sprint. It’s certainly not easy, but you can do it and the docs and dietitians are there to cheer you on the entire way!

Losing 82 lbs. was my best experience. Being overweight and made fun of most of my life saying to stop eating. Obesity is a disease. Form health helped me look at this disease and battled it with their help from the dietitian and doctor, plus classes. Thank you Form Health

I'm leery about the practices of this company. I had begun the process of filling out forms from Form Health on the 14th of August. I was contacted by Krista Reese in a text on September 19th, 2024. I had provided my HSA Bank debit card number, didn't authorized a charge of $199.00. How can a company charge you for services not rendered. I've sent two emails and left messages and haven't heard back from any representative with Form Health. If this matter isn't resolved soon, I will be contacting the Massachusetts Secretary of State, the Massachusetts Department of Insurance and the BBB in their area.

Signed up for FormHealth August 26th. Had to wait until October 8th for an appointment. Now they have moved the appointment to November 12th. Of course they took my $199 but I have nothing to show for it.

It was kinda difficult to hint my wife that she has extra weight, but fortunately she got it on her own without my participations. So, we started to search for various service where she could lose the weight. I faced this one and proposed it to her. She knew what she wanted and thus I didn't participate in her cooperation with the managers from this company. Nevertheless, everything was okay, she told my that she received a training program and special recommendations. I am writing this review in order to express my gratitude to this service as you helped her to lose some weight and these days she still does it.

The team is very sincere, not judgemental. So encouraging! I should take advantage of the files available through Form, but with a full-time job I’m trying to focus on my own exercise and food intake.

I have enjoyed and am very appreciative to the doctors involved. Form is really good about getting informative information to you and really try to help you achieve your goal. The side effects for me have been tolerable so far and i hope to continue this journey with my goal in mind.

I cannot count the number of times I have tried to lose weight in the last twenty years. I could take off 10 lbs, then I would regain 12 lbs. it was a vicious cycle. Then I found Form Health. Working with this program has been transformative. I started the program in March, 2024 needing to lose weight for health reasons - high blood pressure, high cholesterol, raging reflux due to a hiatal hernia and I was pre-diabetic. I was hesitant to start a medication that might aggravate my reflux, so we started with nutrition, exercise and improved sleeping habits. Working with a dietician, I came to understand my nutritional needs (I wasn't eating nearly enough protein). With my reflux under control, I started on Zepbound in May. I have lost 35 lbs, my cholesterol is down from 239 to 146, I am off blood pressure medication, my AIC dropped by a full 2 points, and I have more energy than I’ve had in twenty years. Most importantly, I now know how to maintain the weight loss.

I have a very chaotic schedule, the team work with me to ensure I was accountable. I had appointment early in the morning and late at night. I was able to keep the pounds off.

Convenience is key! You can meet and with your specialists at times that work for you. The specialists are knowledgeable and responsive.

Be prepared to wait for an appointment. It took forever for them to get my medical info and schedule first appointment. First appointment was scheduled for 3 weeks after they got them medical info. Then a few days before my first appointment they moved it out another 3 week. Now, I a week prior to my appointment I have to change the appt for an important work meeting and earliest option is a month after! I signed up end of July and have yet to talk to a person.

Everything was going well with my original doctor but ever since she went on maternity leave, I feel like my needs have not been met. I was given 2 months worth of the same dosage for weight loss even after I explained that I had already been on this dose and would need to go up. And, as expected, my weigh loss has decreased to only 3lbs in the last 30 days. (I was previously on a higher dose and still have a minimum of 30lbs to lose). I can’t get a sooner appt with the new doctor and now I’m not getting responses regarding the appointment or my concerns from even the liaison. I never had theirs trouble from the Push app Drs and if it weren’t for the Rx discount, I would switch back in a heartbeat.

Provider failed to show up for appointment and then blamed me for making appointment outside of normal hours. The appointment was made 7am on a Thursday for an appointment Friday at 8:30am.

Too many reminders via the app, email and text is way too much and VERY annoying
