I have tried to cancel my membership which I decided not to use. I gave them my cc and keep calling and cancelling but I am still getting charged. HELP

Many of us have been engaged in the contradictions and obsessions involved in the cyclical diet and exercise culture for many years. I’ve been on a number of programs with varying success, but the best program of all is FORM because it is doesn’t use the language of restrictions and rejects the mythology of “calories in calories out” and instead treats the whole person - coaching re the type of mindset it takes to engage in a healthy, long term way to release weight by providing support group options, recipes, nutritional classes, and two monthly meetings with a registered nutritionist and dietician and a once monthly check-in with your Doctor who helps you manage medications and/or advocates judiciously for your pre-authorization for those medications, if applicable. In some of the competitor programs, you may, if you’re lucky see a physician once a year and you certainly don’t hear back away from customer service right away as you do with this program!! I was so used to gaps in customer service, long wait times or no responses at all from other competitor programs that I was shocked to find that I received same day responses inside my app from the team! Like many of us - I lost access to my weight loss medication after having success for over a year! That was about nine months ago, but I was able to work through a bit of expected weight creep because of the support from FORM. Also their ability to get medication when it was available specifically in my case, Mounjaro. rebranded and reissued for weight loss, as Zepbound was clear and the timeline for how long we would be waiting was completely accurate, but their encouragement through that process of waiting for meds for nine months!! kept me from going back to old habits like stress eating. There is a common theme out there that if you come off your weight loss medication, because, of course, obesity is a disease and there is an argument to if it’s a disease we need medication perhaps access to it for a lifetime, however, there are exceptions to that rule, and you don’t necessarily have to gain all your weight back when you come off medication. The gentle encouragement from the team at FORM made all the difference in the world, because I had less stress about my weight and the scale in general. I’ve only gained back about 20 pounds from the original 70 pounds I’ve lost before coming to FORM and have stayed in “onederland” largely due to their ongoing support and encouragement. My doctor there was so amazing in helping me understand that I was actually part of a small group nationally to only have gained back 15-20 pounds after losing medication access. She gave me regular encouragement until there was access to medication again which just happened this month after months and months of waiting. I also had to deal with a family emergency during this time so the recipes I got from my wonderful nutritionist and support from both my nutritionist and my physician enabled me to stick to a healthy workout, eating and mental health plan even during a very stressful time as I dealt with a family crisis and the anxiety of waiting for my medication to be approved again. The nutritionist, physician, and support team are stellar. I recommend this program highly and I’m quite sure I will be able to complete all of my weight release goals by working with them patiently and enthusiastically. They have a great team here you can do no wrong by joining and unlike other programs in which I had a high out-of-pocket fee, as long as I weigh regularly w the scale they provide, this program is covered by my insurance company. I’ve been working with FORM many months and I couldn’t applaud this team more. This is a genius program that understands the value of emotional health combined with solid nutritional tips to serve you over a lifetime. They also care great deal about you. You are more than just a number or a $ to them.

Can’t stop raving about the FORM Health program. My medical doctor and registered dietician are wonderful - helpful, inspiring, motivational, all done in a caring, non-judgmental way. The app is great, the resources helpful and the food photo log provides accountability. Plus the ability to chat with my team anytime I may need to. The Zoom offerings are engaging and educational, of great benefit. Dr. Maldonado’s “goal setting” presentation should be required viewing by everyone. This program is the real deal. Down 42 pounds in 2-1/2 months, have a ways to go, but this time my optimism for success is at an all-time high. Thank you FORM Health for leading me down this journey to a healthier lifestyle! OFFERED TO SIGN UP FOR $99 AND THEN WHEN I WENT TO DO IT UPPED IT $199. IT IS A SCAM!

I filled out all the paperwork and paid the fees to find out you have to wait 2-3 months before you can even make an appointment with a doctor. Haven’t gotten to the point of even making an appointment so it could be even longer. Not a good system.

Terrible experience. Paid $199 and am now totally unable to set an appointment or communicate with Form. I feel like they have taken my money and provided absolutely nothing in return. I am extremely frustrated. The response to my review suggested that I email them to address my concerns. I have done that multiple times and received no response whatsoever. I am not sure what else to do. It is simply impossible to communicate with Form.

UPDATE: Form reached out and was able to get me scheduled quicker than previously mentioned. Customer service is quick and solved my problem. Paid the $99 then get an email saying I’ll be seen by March. This should have been said up front if they’ve got a rise in customers. Will this impact me being able to get refills later? I just hate getting I to this program and being put on the back burner. I need help NOW.

Excellent, sound medical advice and nutritional support! There are no secrets here - if you have your mind in the right space to lose weight in a healthy way - the team at FORM is a great partner.

No not even close A week goes by you get ads. You ask for a call back. No one calls the. You find an appt is months out? I can do better with my primary care and a 20 office Visit. The value SHOULD be ASAP Script Done

Waste of time. I signed up, spent ages filling out all the forms and signing loads of paperwork. Then a week later got a blunt message saying I wouldn't be scheduled with a doctor until at least March. I said this is unacceptable and wanted a refund and they said "great, see ya!". Don't waste your time!

You pay the $99 and then you don’t hear from anybody except to receive an email stating you won’t be seen by an MD until approximately March. I have sent messages for a week now and still no responses. This is very deceiving especially when at sign up you complete all they ask for and are told you would hear from a doc and dietitian within 24-48 hours. I want a refund and will take my business to a company that VALUES their customers enough to respond to them.

I love the online appointments because it makes it so much easier to fit into my schedule. I have lost 20 lbs in 3.5 months. Easy to follow plan.

My nutritionist Emily is very proactive & encouraging, although I’m gaining weight because I can’t get access to my previous GLP-1 prescription. Since I’ve been off of it, I’ve gained nearly 10 pounds in 6 months :-(
